If you wish to become a member of Middlewich Heritage Trust please download the membership form to your PC and fill it in and email it to us or print and complete the form and post it to Middlewich Heritage Trust, Victoria Building, Lewin Street, Middlewich CW10 9AT
If you can do neither, please send an email which includes the details requested on the form.
If you are a UK tax payer and want the Trust to claim gift aid on your subscription (at no cost to yourself), please include the words “Please treat as Gift Aid all past, present and future donations that I make. I confirm I have paid or will pay an amount of Income Tax and/or Capital Gains Tax for each tax year that is at least equal to the amount of tax (25p for each £1 donated) that all the organisations to which I make gift aided donations will reclaim on my gifts for that tax year” in your email.
